Care coordinators offer extra time to support patients through primary care. They advocate for specific groups such as the elderly and frail, cancer patients and those experiencing mental/emotional health issues using a personalised care and support approach.

They assist people in being aware of and accessing relevant services to enable them to understand and manage their own health and wellbeing.
