Register for Online Services

To register for online services you will need to complete this form. We will then contact you via email/text message to advise you once you have been registered.

You will need to collect your registration paperwork from reception at Greenhill Health Centre, please bring a form of photo identification with you upon collection.

Once you are registered you will be able to use the service to:

Order your repeat prescriptions
Make an appointment
Cancel an appointment
Change your contact details
Review your medications and known allergies

If you require access to your Detailed Coded Record, a further request form will need to be completed and submitted once you have an active Patient Access Account.
